LOCATION: Conveniently located in Downtown Prior Lake in the Main Office Building, on the 2nd floor in Suite #3.

PARKING/ACCESSIBILITY: Ample free street parking is available as well as free parking lots in the area. There is a handicap parking space located in front of the Chamber of Commerce.

***This business is on the 2nd floor and is accessible by stairs only. There is no elevator.

COVID 19 PRECAUTIONS: While masks are not required, you may wear one during your session if you'd like. Contact me via email, text, or phone call prior to your appointment if you would like me to wear a mask during your session. If you feel ill or have tested positive for Covid19 please contact me to reschedule your appointment.

PRIVACY: A health history intake form must be filled out prior to your first appointment and updated every year or when there are any significant changes i.e.; surgeries, major illnesses, or injuries. You are responsible for notifying your therapist of these changes before the start of your next appointment. The information in your file can only be released with your consent or as required by law.

MINORS: I do not work on people under the age of 18 even when accompanied by a parent or guardian.

PAYMENT: Payment for massage services are due at the time of service. Form Massage and Bodywork accepts cash, checks, all major credit cards and HSA/FSA via Square. For your convenience, prepayment is available online at the time of booking or anytime before your session by logging into your MassageBook account. You may also pay in the office at the time of service using any of the above methods. All sales are final. no refund will be given.

TIPPING: I charge a flat rate for my time and expertise and tipping is not necessary or accepted. If you feel that you received excellent service, I'd appreciate a positive online review or a referral :)

INSURANCE: Form Massage and Bodywork does not offer direct billing to insurance companies but is happy to provide an itemized receipt for the session that the client can submit to their insurance for possible reimbursement. The client is responsible for contacting their health insurance provider to verify this is an accepted expense, and if a physicians note is required.

CANCELLATIONS/LATE ARRIVALS/NO SHOW: Clients are asked to please arrive for their appointment 5 minutes before the scheduled starting time. This allows time for the client to use the restroom and discuss goals and preferences for the session. In the event the client is running late, the service may be shortened to maintain the schedule, and the original treatment time charged.

 

I understand and respect the hectic schedules of clients. When given sufficient notice, appointments may be cancelled or rescheduled. Please understand that operating hours are limited and I am often booked in advance. Appointment reminders are sent to clients 48 hours prior to the session via text and email to give ample time to make any changes to the appointment. If you must cancel or reschedule with less than 24 hours notice you can only do so by reaching out to me directly by phone call, email, or text message (texting is preferred especially for same day cancellations).

In the event that a client fails to show up for their scheduled appointment without giving notice either intentionally or by simply forgetting about their appointment they will be charged the full amount for the session. Payment for a missed appointment will be collected by either charging the card on file, redeeming a prepaid package session or gift certificate, or sending an invoice via email. If an invoice for service cost is sent, it must be paid before any future appointments can be booked.

INAPPROPRIATE BEHAVIOR/REFUSAL OF SERVICE: Form Massage and Bodywork has a zero-tolerance policy for inappropriate behavior, jokes, or comments of any kind. In the event that this happens the session will end immediately. The client will be charged the full amount of the session and asked to leave and no future appointments from that client will be accepted. I reserve the right to refuse/discontinue service to anyone at any time. For privacy and comfort, draping is required during the entire massage. No exceptions.
